(Download a free version of Acrobat Reader DC here.) You may fill the form online and then print and sign, or you may print the enrollment form and fill it by hand. To enroll your child, you will also need a current proof of address and guardianship. Once school opens you will be given several other forms to complete.
Once your child is enrolled, you may contact your child's school for instructions on how to obtain an activation key to the Infinite Campus parent portal. This free online service will permit you to fill most forms online rather than using paper forms.
The Truancy Mediation Program
According to school board policy, Nelson County students are required to attend school regularly and punctually.
Any student from age six through seventeen who has been absent from school without a valid excuse for three days or more or tardy without a valid excuse on three days or more is a truant. (A student who is absent for thirty-five percent or less of the regularly scheduled school day is tardy.) Any student who has been reported as a truant two or more times is a habitual truant.
When a student is declared a habitual truant, the parent may be notified by a certified letter and/or by a personal visit. If truancy continues, both parent(s) and student may be required to attend the Truancy Mediation Program in which reasons for truancy are discussed and solutions for truancy are formulated.
As a final recourse, court proceedings will be instituted if truancy continues.

The Home/Hospital Program
The Home/Hospital Program is a service to students unable to attend regular school for medical reasons. The Home/Hospital teacher serves as a liaison between participating students and their classroom teachers. Assignments are gathered and presented to students at a minimum of two visits each week with at least one hour of instruction each visit. The program is not designed for long-term instruction but for relatively short periods of time. An application process must be completed with the attending physician participating. Application forms may be obtained at any district school or at this link.
Director of Pupil Personnel
The Director of Pupil Personnel (DPP) seeks the enrollment in school of all students who should be enrolled and attempts to keep those students in regular attendance. The DPP must ascertain the causes of irregular attendance and truancy, be in contact with the parent or guardian of the absentee student, and seek the elimination of the causes. The DPP acts as a bridge between the home and the school, acquainting the school with the home situation of a truant and the home with the work of the school.
The DPP’s office manages demographic and attendance records for the district. It also formulates district procedures relating to the Infinite Campus student management software program, including IC Messenger. The office submits all required attendance reports to the Kentucky Department of Education and is also in charge of the transfer of all appropriate records to an electronic format.
